I totally agree, a wonderful touching movie.
I think that rather than being seriously mentally ill Grazia is merely trying to cope with the restricted lifestyle she has on the island.

missed entire point!
Hi, I thought the movie was overall pretty darn good. It seemed like the reviewer missed the whole point...
I understood that the villagers needed to seriously lighten up - to enjoy life and not take for granted all the beauty around them. They needed Grazia to show them how to live.
Yes, Grazia most definitely had what western medicine would today diagnose as "manic depressive," but remember - people have been having these problems for years and living with them - getting through life without today's western medicine.
I think her condition could have improved by staying right there in Lampedusa. Maybe if the people of Lampedusa weren't so alarmist/uptight and trusted in themselves and their region's wisdom (again, a way of appreciating their natural beauty) they probably could have helped her lead a more balanced life.
In turn, she would give them what they needed - an zest for life.
And, there isn't necessarily a need to take everything so literally - it's all symbolic. It's based on a fable after all, right?
I give it an A!
