I am also someone who just watched the movie and is quite confused - so looked for explanations online. I also find it odd that IMDB and Wikipedia and a few other reviews don't address the confusing nature of the movie - so I'm not certain if its a subtitle problem.

I think that seungmin has to be the killer. At least one of them. Son-yeon finds out... and then they act together. The motive for seungmin is that she believes son-yeun should be avenged for what son-yeuns father did to her. Seung-min loves son-yeun like a sister and would do a lot for her but at the end when she was a suspect she panics and wants to stop. son-yeun is an eccentric artist - she finds the whole sewing up and blood gore artistic and doesnt want to stop. At the end seung-min panics because she knows the police are onto her. im not sure whose blood it is that she splatters on the wall.if its her own then she wants to frame son-yeun for attacking her and then kill her as a self-defense thing. if its son-yeuns blood she is splattering (how did she get ahold of it? when she was in the hospital? is that why she was so tired in the hospital?)then it is to fake a murder scene. perhaps she wants to run away with son-yeun. the police will examine the amount of blood lost and determine that someone could not survive with that amount of lost blood. And pronounce her dead assuming that her body parts are in a plastic bag somewhere.

But son-yeun is crazy and selfish and does not love seung-min the same way. Hence kills her and she appears completely innocent. However 'the work of art' is still incomplete. She wants to complete it because she is an eccentric artist. the poor plane guy she lied to (or told the truth to??) was perhaps the head of the next victim. she said she would 'be back' - possibly to implant the head of her next lover onto the sewn body parts.

son-yeun spared cho cos he remained professional with her through out. hence she had no need to cut off his head because he wasnt a lover.

she didnt think anyone would see the sown bodies because the house is in a remote area and everyone thinks she is innocent.

Not sure if i havent addressed some part of the dilemma. I probably haven't...

oh- the significance of the photo: perhaps that all of the victims and the girls knew each other and met at the house. hence being caught in a lie because she said they didnt see each other anymore / hadn't seen each other.

i dont understand the significant of the fish - perhaps the idea of trapping people in a tank stems from it.

as for the orphelia pic.. NO IDEA. perhaps just to reiterate that she is a crazy mad genius artist

Re: i think someone needs to TELL ME SOMETHING!

lots of good theories thus far...
    1. i agree that the body is that of her su yeon's father AND of the parts from the ex-boyfriends all sewn together. in her anger, insanity, hatred or whatever, she does this. the"why" is comprised of just the psychological trauma she has.
    2. the picture of the woman in the water is symbolic of the her feelings throughout the trauma, or of her sanity...or both. in the plane scene, she is starting fresh, walking away from the trauma. she gets away with things because of how innocent seeming she is (basic instinct-like??)
    3. the doctor is the one slicing and dicing. she has been so entranced with su yeon and has always tried to protect her. she may have been covering up for su yeon (reason for blood smearing) or may be trying to help su yeon still by putting the blame on herself or perhaps su yeon asked her to do something to make su yeon appear innocent or any of the other theories...but did anyone think she had the look of someone betrayed when she got shot?? su yeon cried though, despite that fact that she had to have planned everything, because she was losing a friend. but she knew she had to let that go and tie up all the lose ends.
    4. su yeon lied about the neighbor being a boy. this is yet more evidence of her hiding/lying/guilt. it just doesn't dawn on the detective yet...
    5. the detective reacts the way he does not only because he royally got screwed but because i think he realized that his head should've be oin that tank....
    6. because things no longer went the way they should have (with the detective's death), its possible that su yeon had to improvise or end up being found out, which unfortunately led to her losing her surgeon, which then meant that detective guy gets to survive.
    7. everyone su yeon gets involved with/uses/intends to kill gets a glass with liquid in it...
