The doctor (who was the neighbor) did not "become female". When she was young, she was a tomboy. So Chae Yeon thought that she was a boy, but she was actually a girl. It is not clear if Chae Yeon ever knew that her doctor friend was her childhood neighbor. blood on the wall in the bathroom and left her
What i don't get is why the doctor sprayed the handprints all over it, before she went to meet Chae Yeon in the music store. Did she know that Chae Yeon was the murderer?

Very interesting theory.
But why has the doctor (not nurse) become a female? And still . . . what are her motives for killing Chae Su-Yeon?
