Very interesting theory.
But why has the doctor (not nurse) become a female? And still . . . what are her motives for killing Chae Su-Yeon?

This movie has one of the strangest endings ever. But has anyone else come up with this: The nurse who is Chae Su-Yeon's friend is actually her brother, the one whose pants caught on fire when he and Chae Su-Yeon were children? Notice the nurse's legs when she puts on her stockings. They are scarred. So, she tries to murder Chae Su-Yeon because she knows that her sister is a killer. That is why she grabs Chae Su-Yeon in the music store and puts a knife to her throat. But her attempts are in vane because the police think she, the nurse, is the killer, and Chae Su-Yeon the victim. The killer goes free.
